That, but also based on what google thinks is the most relevant for you. Take e.g. a word like windows - google might think you are referring to the OS or the pieces of glass in your wall depending on whether you've shown an interest in software or home improvement stuff before. Just as a super simple example.
Search engine optimization are basically techniques to try and make your site seem the most relevant to google's algorithm. And yeah, it costs money in the sense that you need to pay an expert in this stuff to do it for you if you want it to be effective. Although I don't know how well it works now, since I think google was making some efforts to minimize the impact it has on its results. No idea how that turned out, I just remember vaguely reading about that.
