That is a set from the contest, and one of the requeriments was creating a 3 items set. So either the creator either intended the undershirt to be part of the top or the hat to be part of the hair (which seems less likely) otherwise you'd have a 4 items set.

1.No, it would not have been possible, as undershirts only work if they are skintight.
2. Ninja Ai designed this as one item, not two.
