excellent, thank you. I've emailed those to myself as a backup, so now I can't lose them! :) I think my prescription is out of date. Do they have to see it/learn the exp. date or can I fake it (because it's only BARELY out of date and still correct according to some self-tests at the DMV)?

I'm pretty sure they never ask to see the actual prescription; you just enter the fields in manually.
I've been using the same one for 10 years or more and they've never requested that I update it or send them anything official, so it shouldn't be a problem.
